Your best bet is to check with the airline; they will have regulations about how cats may be carried in flight. There will also be a potential problem with bringing a cat across an international border; while I would normally suggest checking with the US Border Service, I suspect that your airline will have the necessary information on that as well. You will particularly need to know if there is a quarantine requirement.
Yes I would take a map with you if you do not know the area in Arizona or where you are going so you do not get lost. If you know the area then you will not need a map, but if you do not take a map of the area with you.

One in Arizona i don't know whats its called but i really need to know for my test!
You can have one but you dont need it.
It depends on where you are coming from, what airline, and where you are landing in the DR.
I think you need it in every urban lake but i dont know if you are allowed to fish there?
You would need to know how fast the plane is flying to calculate that.
You can't redeem airmiles for one-way flights. Round-trip only. I know, it sucks.
I really need to know that as well. I can't get two answers that are the same!

No, you do not need a passport to travel to Arizona if you are a U.S. citizen.
