It's not an extra toenail. It looks like one, but it's really a corn. It's called a "Lister Corn" and it develops on the outer corner of the nail, usually from pressure and friction on that spot. The main cause is that by the time most of us reach adulthood, our toes have become slightly deformed from a lifetime of walking in shoes. Stand up and look straight down at your feet, you may notice that your pinky toe has curled so that you're putting a lot of pressure on the outer corner of the toe/nail.
I don't think there's much treatment for it except moisturizer and exfoliating...if it's really severe, I guess they can do surgery to fix the shape of the toe.
It is only a toenail, there is no reason to be scared of it! Some people find relief from ingrown toenails by cutting a small "v" shaped wedge out of the nail in the centre of the affected toe. Checking that your shoes fit correctly also helps.
