The treatment for heaves centers around prevention of exposure to irritants and control of inflammation and mucous production in the bronchi.

For heaves that is associated with stable situations (mold, fungus, dust) then horses should be turned and not stabled, hay should also be served soaking in water because it is typically a source of serious irritation. For heaves associated with summer pastures/blooming plants then the horse should be kept in a dry lot area to reduce exposure to the pollens and spores from plants and again, hay should be served soaking in water. Prevention is key because each attack can do permanent damage to the bronchioles and make each subsequent attack more serious.

For medical treatment, typically a combination of steroids and bronchiodilators are used. This is because you need to treat inflammation, mucous production and the constricting of the bronchioles. The drugs chosen will depend on how serious the attacks are and what routes of administration are available to the owner. Treatment needs to be initiated as soon as an attack is noted because it take about 3 days to completely stop the adverse events occurring in the lungs and the longer and more severe these events the more risk of permanent damage to the lungs.

Emphasyma (in humans) is from second hand cigarette smoke. If a horse has emphasyma it may be from inhaling smoke over a long period of time. But what is likely the case is that the horse has what is called the heaves. An unfortunately incurable problem much like chronic bronchitis. The heaves is caused by lung infections that have been left untreated and that have destroyed the cells in the lungs beyond repair. Try researching the heaves in horses and see where that gets you.
