If you are playing on Hardcore any companion can be killed, Rex included. Otherwise, companions will simply be knocked unconscious and can't be killed. If you never finish the quest Nothin But a Hound Dog, I believe Rex dies as part of the end game sequence. I've never not finished that quest though, so I'm not sure.
