No one knows. It's like asking where did the flu, toenail fungus, the bacteria that causes sinus infections, and a trillion other infections originated. STDs have been with us since before humans could probably talk to each other.

There is nothing "dirty" about having an STD, as probably one in 4 people will have had one at some point in their lives. Take the stigma out of it - it's simply an infection that can be treated.
it is more of a guess where it came from, the origins of it were never were discovered, but it is believed to of come from the Black Plague era.
There is no way to know where and when the first case happened. There are writings in The Bible that describe illness similar to gonorrhea.
The answer is not knowable. There are records of similar disease from biblical times, as well as in French and English legal documents.
