Not in the US. Using Mr. or Mrs. with a first name only (like Mr. Fred or Ms. Judy) was originally the way slaves were instructed to speak to their masters as a way of showing deference. Today, it is practiced only in the South as a lingering remnant of racist practices. It is appropriate to address someone named "Fred Jones" as Fred if you know him, Mr. Jones in a business setting, or Mr. Fred Jones in a more formal or traditional setting. Addressing him as Mr. Fred is considered derogatory and disrespectful.

Is there a space after Mr and before the first name?

In English Grammar, there should be a full stop/period after 'Mr'. because 'Mr' is shortened form of 'Mister'. After the full stop.period then a space then a capital letter to start the name. e.g. Mister Joseph Smith , in shortened form it is 'Mr. J. Smith.'. Note the stops and the spaces. NB For any word in the English language that is shortened, then the shortened form has a full stop/period after it. e.g. Mrs. (Missis) Dr. (Doctor) Capt. (Captain) No. (Number). # & e.g. 'Exempli Gratia'.
