In the address line, send to her home or business depending on what capacity you know her in. If you're sending with flowers, fruit, etc. make sure you're sending where somebody will be there to sign.

Notes should be handwritten and hand addressed, and the card should either be a generic or religious sympathy card (if you know their faith). General religious cards such as "Your family is in my prayers during this time of loss" are usually appropriate whether or not the person was religious.

If you knew the decedent well, you may want to use a nice piece of stationary or a blank card with flowers, etc. on the front to write a letter expressing good memories or stories. If you have photographs or keepsakes you want to present to the widow, it's usually best to wait until after the funeral and send in the mail.

It is always appropriate to use a person's name. Address the letter to Mary Smith. Her connection to her departed husband will be mentioned in the body of the letter.
