A letter to a bank manager would count as a formal letter and as such you should adhere to the standard protocol for writing a formal letter (for example, starting with 'Dear Sir/Madam' and ending with 'Yours Sincerely'). In this case, it would also be important to remember to include all the details necessary to transact your business.
To write a letter of dispute and bank fee be sure to include all pertinent information. Include bank account number, name, address, billing statements, what you are disputing and address them Dear Sir or Madam.

"Dear Sir," is all you need to use.
Either you write out the same letter multiple times with different addresses, or you write the letter, leave space for the address, photocopy it however many times you want and write in the different addresses later. That or e-mail.

Well I just have to say this why would you write a job application letter if you dont know the job that you are applying for its totally poinless and that is all I have to say.
