It means to leave someone. It can be permanently, as when leaving a relationship, or just abandoning someone for the moment, such as at a party. Ditching school means you are somewhere else other than in your classes.

Examples:

"I thought our relationship was fine, but then she decided to ditch me and hook up with someone else."

"I had to fight two people alone since my best friend decided to ditch me after they started attacking me.
