This is an idiom that refers to not being willing to change. It can refer to behavior, or to emotions: a counselor might say this to a client who is afraid to do something different, even though doing the same thing is causing problems. You may also hear a version of this idiom in business-- to "think outside the box"-- in other words, to go beyond what is typical or usual, to imagine a strategy that is different from what has been done so many times already.
