Sort of. Most of them count the number of revolutions of either the supply (left) or take-up (right) tape spool. Let's take a counter on the supply spool. As you play a tape, the size of the tape pack changes from its maximum, down to the size of the actual reel hub at the end of the tape. This means that the number of turns of the supply reel - per (roughly) 9.375 feet of tape and thus per minute of playing time - is changing all the time. The counter will go slowly with a full tape pack (large diameter), faster as the tape is played (small diameter). As well, tape thickness depends on the cassette's playing length. A C60 uses thicker tape than a C90, which uses thicker tape than a C120. As well, really short tapes (say, C46) probably use the same thickness as a C60, but put less tape into the cassette.
