At 12 weeks is fully formed, but very, very small (too small to survive). He or she already looks like a proper baby. The baby is only 2 inches (5.5 centimetres long) and weights 14 grams (0.5 ounces). He or she already has eyes (which are fused shut), ears, a liver (that is already creating bile), kidneys (which are already producing urine), a bladder and basically everything else. His or her heart has been beating for around 6 weeks already and the heartbeat is visible to hear and see on an ultrasound. He or she will squirm if touched because he or she already has nerve cells and is starting to be able to feel pain and other sensations.
you're further along than 4 weeks. think back, you're more likely at least 8 weeks along, closer to 12.

This is normal for babys. Some infants do get acne when they are 6 weeks old. Any earlier or later, you notice acne on your child's face is fairly unusual. Don't bother washing the babys face. It will clear up on its own by 7-8 weeks of age.

Twelve weeks is not exactly equal to three months, as months vary in length. Typically, three months would be about 13 weeks, considering that most months have around 4.3 weeks each. Therefore, while 12 weeks is close to three months, it falls slightly short.
