I can't go back to 1950, but I vividly recall a visit to an eye, ear, nose and throat specialist with my mother - I was a young teenager - and the cost for the visit was $13.00. My mom paid as we left. No insurance - just a market transaction charged at a rate the market could bear. I just went to a urologist the other day and got a "discounted" bill for $370.00. The insurance company had forwarded it to me because I have a high deductible (by design). The market relationship no longer exists, however.
