What factor is not included when determining the level of service for E and M codes?
E&M services are usually based on the levels of History, Exam and Medical Decision Making. However, the final E&M code is also based on the nature of the presenting problem, counseling, coordination of care, and occasionally time. There are specific rules about leveling your E&M service based upon time and, along with this, there are specific rules concerning documentation when coding E&M's based on time. In order to have a proper understanding of this, I recommend reading the guidelines set forth in your CPT Coding Book in the section labeled E&M services.
