Physicians do not typically measure drunkenness. It is law enforcement officers who generally obtain estimates of blood alcohol concentration (BAC) by using breath alcohol testing machines. These devices can only estimate BAC indirectly through analysis of exhaled breath. It is technicians who generally measure BAC directly through drawing a sample of blood and testing for its actual (not estimated) BAC.
BAC is not a direct measure of drunkenness because some people develop tolerance to alcohol and therefore will not be intoxicated at .08 BAC. On the other hand, others may show signs of intoxication at much lower BACs. Therefore, identification of drunkenness is somewhat arbitrary; it is a social and legal rather than scientific concept.
