Doctors must do careful retirement planning like most other people. Some doctors may be fortunate enough to work all or most of their professional lives for large medical centers, and may receive retirement benefits from those places in the form of pension plans that the centers have set up. Other doctors may have private practices or work for smaller hospitals or clinics. Like most other people, they have to save and invest so that they will have enough money to generate investment income during retirement.

Thinking of it this way might help to clarify. In general, doctors are paid through medical coverage that patients have, through private insurance, various state sponsored programs or through things like medicaid or medicare. These sources of payment for medical services rendered are not geared up to continue to pay physicians after they have stopped seeing patients.
