The cells of a human body are generally slightly salty. Therefore, by adding fresh water, we have the action of osmosis happening, but if the doctors used plain water, the cell would fill up too much because of the excess water in the vacuole (a place the cell uses to store water and other necessities) and it would lead to the cell bursting and dying. Also, since the cell doesn't have a cell wall, the cell can burst like this, unlike a plant cell which has a cell wall. Technically all this means that it is very bad for the person when you use plain water. :)
