The three worst by casualties would be Antietam, Gettysburg, and Chickamauga. As far as just being horrible, I would propose 1 - Spotsylvania Courthouse, The Wilderness, and Chancellorsville (with Cold Harbor and Fredericksburg runners-up.
Spotsylvania featured nearly 20 hours of bloody hand-to-hand combat at the Bloody Angle in mud and rain at horrible cost to both sides. Both the Wilderness and Chancellorsville had terrible casualties, but the death of the wounded in wildfires in the dense brush is particularly awful.
In response to the above, 1st Bull Run was a farce. I think by "bloody ally" is meant the Bloody Alley at Antietam, which battle does deserve consideration. 2nd Bull Run was bad; there was a dreadful short-range stand-up shootout that lasted at least two hours and resulted in one of every three men engaged being shot. But I think on balance the "worst" would be those listed above.
