To remove the interior from the door you need to remove all the screws (including from behind the speaker cover). Once this is done the door card will not budge. The whole door card is glued on with an incredibly strong adhesive. I used a hacksaw blade to gently remove the glue. The glue is only on the bottom half of the door card.
