"V" represents the opposing positions of the cylinders, just like a V-8 engine. The bottom of the "V" in the location of the crankshaft and each leg is the cylinders. "Twin" simply means there are 2 cylinders on each leg of the "V". Therefore a "V-twin" is a 4 cylinder engine.
