While many people think Shax, a bounty of the Source, went and killed Piper, but Phoebe made a deal with the Source to rewind time and save her. He did but when the whole scence restarted, Phoebe wasn't upstairs to warn them. Shax killed both Piper, Pure, and their innocent. Leo came just in time to save Piper but not Prue hence, she died.

One reason she never returned is one of the reasons of why she quit; she was tired of Holly Marie Combs(Piper) and Alyssa Milano(Phoebe) moaning about their job, Shannen(Prue) wanted to 'work with people who like to work, but not a total bore either' as quoted in one of her interviews.
Another reason was about an alleged argument with Alyssa.

I am very sorry to tell you this but there is no season 9 in charmed season 8 episode 22 all that happens is that you see piper's kids and phoebe and her boyfriend and then you see piper telling a story to some kids.
