There is no official canon story for that, as in the film's makers have never revealed their vision of that. The closest to official story which howver is not what really happened as it was not created by Disney at all and was not approved by the film's makers, is in one of the books in The Lion King book universe.

The book A Tale of Two Brothers tells a story how teenage Scar (then called Taka) got the scar from a water buffalo's horn as a herd of them attacked him when he was trying to make Mufasa look bad in their father's eyes but his plan went horribly wrong. That story is not what really happened and Scar's real name isn't really Taka because it's an original storywritten by Alex Simmons who wasn't part of the Walt Disney Company and thus Disney only approved and released the story - and because on the commentary track of the film, the film's makers denied that book's main character Kopa's existence in the movie and through that everything else about the books. There is no official story about what really happened.
