He hit himself in the chin with his whip
In the movies, Indiana Jones received the scar on his chin when he was a teenager. In the beginning of the Last Crusade, you will see that he used a whip for the first time, and wasn't very talented in using it. When he whipped it out, he accidentally lashed his chin with the end of the whip, thus giving him his scar. In real life, Harrison Ford has explained the scar by relating the story of getting into an accident while driving his first car. He said that while driving his first car back when seatbelts were still new, he reached back to pull the shoulder strap down, became distracted, and swerved off the road and into a telephone pole.
Yes, he had a cameo in Indiana Jones and the Temple of Doom. No, he is definitely NOT in this movie.

In the beginning of Indiana Jones and the Last Crusade(1989), young Indiana is running on the top of train cars. He falls through one of them and encounters a male African lion. He uses a bullwhip to fend off the lion. The whip smacks him on the chin and gives him his famous scar. However, this is not the whip he uses in the various scenes of each movie. The scene only serves to introduce how he came about using a whip in the earlier movies. There were many different kinds used in all of the movies.
