In Platinum, you have to obtain a Secret Key (which can only be obtained through the Mystery Gift event) and go to Rotom's room in Galactic Eterna Building and you'll find a 5 appliances. If you want the Fan form, choose the rotary fan.
In Heartgold and SoulSilver, Rotom's room is located in Silph Co., which can only be accessed if you enter the broken elevator with Rotom as your walking Pokemon.
In Black and White, you can change his form in the basement of Shopping Mall Nine (in the boxes to the right).
