This trick is called "The Human Blockhead" for obvious reasons. Some performers have been known to do this with hand or power drills, screwdrivers, Pens or pencils and a multitude of other things.

This is NOT something to try on your own. The nail used is not a trick nail and putting it in at the wrong angle or too far could cause serious irreversible health problems.

The easiest thing to do is mention that it is not an illusion, the nail is real, it does take skill and practice, and you insert the nail into your sinus cavity.
