How does Penny die in Holby City?

Spoilers, for those who have not seen the episode and have not seen it yet, and want to see for themselves, please do not read.

Unfortunately, yes, Penny did die on Holby City today (12/04/11).

Her death has to be on of the most unexpected in any TV program I've ever watched.

The episode focused around Oliver and Penny's ultimatum for him, whilst Oliver tried to redeem himself, there was an accident with a lot of victims, some doctors from Holby had to go directly to the scene to see to it. However, you don't see what goes on there.

At the end of the episode, Michael and Petrenko return to Holby, they question Penny's whereabouts, as she didn't return with them. Michael shortly received a phone call after this concerning Penny.

The next scene shows Malick, Petrenko and Michael walking up to Oliver to say that they need to talk to him about Penny, then you hear Michael voicing over a scene informing Oliver that Penny in fact tried to save someone from the scene and that there was a collapse, and that she wouldn't have survived. After this they tell Oliver that he needs to identify the body, because it needs to be a relative. So then Oliver goes to identify the body and Michael and Malick stand by him, when the sheet is pulled back, to everyone's surprise, it is in fact Penny, and she had died, unexpectedly.

Oliver admits to cheating and swapping his and Penny's papers in order to pass, he confesses this to Malick, and then goes out to the front of the hospital towards Hansen, where there is a news crew interviewing him, but Malick changes Oliver's mind to confessing, and Oliver just sits down with a bag of Penny's property, and the episode ends.
