A laser shines pure light onto the disk. The reflection is picked up by a photo diode or transistor, through the same optics, using lenses and mirrors.
On commercial disks, under a protective clear plastic, bumps are pressed onto a metallic surface. As the disk spins, the bumps produce a signal which can be analysed as 1's and 0's to form digital data.
Electronics controlled, by a computer, take care of the tracking and focus of the laser module, as it moves from the center of the disk to the edge.
