The wives were supposed to have children, if they could. If not her husband was given a handmaid, and the wives were basically just there to maintain the household, that was pretty much it. Also, the wives of poorer commanders had to do everything themselves, aka bear children, and do the job of the marthas.

This is why many handmaid's kill themselves.
