Gibbs shot her
In Season 6, there were 2 episode where Team Gibbs investigated and searched for a little girl who was kidnapped and is Lee's daughter , the guy holding the little girl captive is looking for information about DOMINO and Team Gibbs makes a deal with him, he will give him DOMINO in exchange for the girl. At a bar, where the man is supposed to be we find out that Bankston (the guy who was supposed to deliver DOMINO to the man, who also has Bankston's wife) is the actual man who kidnapped Lee's daughter and steals DOMINO and takes Lee hostage. Bankston shoots at Gibbs and glass shatters and he cant focus his gun on Bankston.
Lee ends up giving Gibbs a nod, meaning to shoot. Gibbs fires his weapon and shoots Lee three times in the chest, killing her but also killing Bankston.

∙ 14y agoshe dies of gunshot wounds in the line of duty. she gave gibbs permission to kill her in order to kill the bad guy that was holding her hostage. gibbs shot her through the abdomen to kill the guy holding her.

As for the other thing, Lee's sister was being held hostage by some people who were watching her, she didn't want them to know that NCIS had figured it out, so that's why Gibbs said they were working together, because they were, they were trying to find a way to ger her sister back.
