Dean dies because of the deal, but Castiel the angel of The Lord carries him out of hell and back to life. To confirm that, Dean has this HUGE hand print on this arm (I forgot which one) and that marks where Castiel grabbed him. He appeared in his coffin in Pontiac, Illinois (I think) and drove to Bobby Singer's house. They later on find Sam (who's with Ruby the demon). Later in the middle of the episode they go to Pamela's house (a psychic) who only finds out the name of Castiel, then gets her eyes burned out. Dean and Bobby later go to a barn and perform a summoning ritual to summon whoever brought Dean back to life. Castiel arrives and introduces himself.
