It is loosely based on the the Soviet Baltic Fleet destroyer torozhevoy ("Sentry"). Do a little research and you'll find the historic account. Not like the Hunt for Red October, but like most Hollywood movies, literary license was taken for big box office purposes.
EDIT: The movie The Hunt for Red October was actually based on the novel of same name, by Tom Clancy. It was the novel that was loosely based on the mutiny of Storozhevoy. When Clancy wrote the novel, it was widely believed that Storozhevoy was going to defect to the West, and he based his story on that assumption. It was not discovered until the end of the Cold War that the ship was not defecting, but actually attempting to spark a second Leninist revolution in Russia.
The other major difference between the novel and the true story was who led the mutiny. On Storozhevoy, the mutiny was led by the political officer, Valery Sablin, while on Red October (Krazniy Oktyabr) it was led by the captain.
