Katherine got a teddy bear from dylans real father but she idnt want Dylan to see so she put it at the top of a bookshelf in Dylans room. (Dylan was young at the time). Dylan saw the bear and tried to reach it but the bookshelf fell on her and crushed her. That is why there is a dent in her room!
