For new construction, mine are:

Standard is 48" to the top of the switch box.
The standard height they are placed is no more than 48" above the floor. I mount my switch boxes with the top of the box 48" above the floor. You can install them lower if you wish but no lower than 15" above the floor.

The blinker switch is located to the left of the steering column. It is bolted up behind the dash board. It is not with the fuse box.

You want the bluebird box to be at least 5' in the air.
Standard is 48" to the top of the switch box.
For a light switch in the home four foot to centre is a reasonable height. Some contractors set the outlet box at four foot to the bottom of the outlet box and other contractors set the box to four foot to the top of the outlet box. There is no code rule that stipulates the exact height that the outlet box must be set to.
As long as its a stable and secure mount, and installed correctly, and of course height restrictions observed, yes, that will work.
Remove the lens, and the mounting screws are at the back of the box.

Check the mounting of the gear box, because the mounting of the gear box is like shock absorb.
First of all if all your lights don't work then its your switch I was told on my 94 it was the switch when my tail lights wouldn't work but all my other lights did. if that's your case or vise versa then its a bad ground somewhere. If not there is some mounting hardware under the dash almost where your fuse box is.
The standard height is around 12" to 15" the bottom of the box above the floor but for the disabled you can install them higher but no more than 48" above the floor. Pick a height that will work for the disabled and install all of them the exact same height.
If it is a rectangular box, then volume = length*breadth*height, where each is measured in inches. If it is a cylindrical box, then pi*radius2*height, where the radius and height are measured in inches.
You can, but you need Wario to break the black box to reveal the switch.
It's good to get a box spring that comes with the mattress which is matched to work perfectly together.
