There were a few problems with the Titanic when she sailed. 1) The watertight bulkheads did not go high enough to stop water from overflowing into the next compartment and pulling the ship down by the weight of the water. 2) There were not enough lifeboats for everyone on board (but there were enough to satisfy the current rules and regulations of 1912). 3) There were Not enough Binoculars for the lookouts in the crow's nest to use. The lookouts did not have a pair of binoculars (even though binoculars for the lookouts were standard White Star procedure) and they had to rely on the naked eyes alone to look for icebergs. If the lookouts had a pair of binoculars in the crow's nest, they Might have seen the iceberg in time to avoid it and escape a collision.

Because they thought it was unsinkable and they didn't know that the captain would hit an iceberg.
There was nothing wrong with the ship until it hit the iceberg. Little did they know that the steel was made wrong, but wasn't known until a few years ago.
