If you are talking about the manga,

AFter Katsura Kotonoha finds out that Mokoto and Sekai are together, she tries to stab Sekai in front of Mokoto but Mokoto steps in front of sekai and takes the hit instead of her and ends up going to the hospital. When he awakes, Sekai tells him that Kotonoha (Katsura) went home and hasn't been seen again. Then, Mokoto gets a phone call from Kotonoha's (katsura) little sister, and she says that Katsura (kotonoha) is on the ground and doesn't move when she calls her, and that there is blood everywhere around Katsura (kotonoha). Then Mokoto hangs up and was about to tell Sekai when Sekai says that she knows about Katsura being stabbed. She says that she went and stabbed her so that she and Mokoto can be together forever without the interference of Kotonoha. FIN

If you are talking about the anime,

After Sekai kills Mokoto for leaving her when she says that she is pregnant with his child and Katsura finds Mokoto dead in his house, Katsura texts Sekai and asks her to come to the school roof at night. She does but she brings a kitchen knife that she used to stab Mokoto in her coat. Katsura tells Sekai that Mokoto wants to talk to her and points to the bag behind her. When Sekai looks into the bag, it's Mokoto's disembodied head. Sekai is throwing up while Katsura takes out a big bloody machette( which she used to cut of his head) and walks towards Sekai. Sekai pulls out the knife but Katsura hits it outta her hand and she is defensless. Then she slices through Sekai's neck. Then she opens up her stomach and looks inside and says, "Just as I thought.....no one is inside you."

The she is seen on a boat, hugging mokoto's head, saying " We can finally be alone together."FIN

They are both really different and that's somthing i hate. I like the anime better personally. The ending isn't the only difference in the manga. So is the storyboard.
