Basically, the guy is forced to take two chains. Behind a screen of glass is his 57-year old secretary, who is also a diabetic grandmother with 3 lung diseases, and his 22-year old Mexican intern, who has no friends or family, but is perfectly healthy, gagged and put upon hinged platforms with the nooses around their necks. If the guy lets go of one of the chains, the corresponding platform will drop, hanging and slitting the throat of victim. The chains will start to retract, making th guy have to choose fast.
At first, the guy is reluctant, claiming he won't kill anyone, but in the end, he chooses to let go of the intern's chain to keep the secretary safe. The intern's blood splatters the glass as the guy curses Jigsaw.
