His ear becomes infected after he dives into freezing water to save his little brother. He eventually goes deaf in that ear because of this. Later in the movie, hearing in this ear is restored when his guardian angel allows him to view the world as it would exist without him. The film doesn't make clear whether the restoration is permanent but it is often assumed to be so.
