During one episode, Cookie Monster enters rehab for his cookie addiction. When cookies are found in his room he denies knowing anything about them and tries to blame someone else, screaming that the hospital employees are Nazis as they inject him with some type of medication to calm him down.

Originally he was just a monster who enjoyed cookies and demonstrated stuff like "slow" and "fast". When parents complained about child obesity from cookie monster's habits, they turned him into a guy who explained that cookies are a sometime food and that you should eat healthy. unfortunately, this lasted for a few months before he became a cookie monster again.
"Famous" may not be the best word to use, but Yes, Family Guy is known around the world.
No. He has never provided a voice for Family Guy. He did however appear on South Park (not voicing himself obviously) being eaten by a monster.

You should try brassmonki.com The guy who makes the shoes is amazing:) They have Bob Marley, Cookie Monster, Obama, etc. He is awesome. They come in men and woman sizes!
All I could find was mention of the line 'you can call me cookie if you like' from the show Family Guy. And 'they call me Cookie' from the movie Stalag 17 from 1953. But the person saying this line seems like an older, soldier or POW.
