Well...in the Disney Animated Feature version, Maleficent is the name of the "evil" character. It doesn't mention her being a fairy...but just that she is powerful. I would assume she is a 'witch' or 'fairy,' because she does not age and she seems to know a lot about how the other fairies work...but really the only mention of the fairies are the three good ones; Flora, Fauna and Merryweather.
Her stepsisters and her step mother.

She didn't have one. Names seem to be appallingly rare in fairy tales. In the 1937 Walt Disney movie the Wicked Queen was known simply as The Wicked Queen , however , in the comic strip version , the Wicked Queen WAS named as Queen Grimhilde!
