From what I can find from various sites, it appears in 1989 Zac Morris from Saved by the Bell was the first show to feature a character using a real handheld Motorola mobile phone.
Maxwell Smart in 1965 used his shoe phone in Get Smart but it wasn't a 'real' phone.
I did find a forum post regarding 1968's The Doris Day Show, evidently a character used a suitcase phone to call Doris in an episode but it couldn't be called handheld.
