The problem was that they were in a hurry to make good time to New York on the ship's maiden voyage and took a risk (of hitting an iceberg) which did not come off. There may have been some over-confidence on the part of the crew who could not believe the ship could possible sink. When the ship sank the problem then was that there were too few lifeboats and too little practice in using them.

One person might say that the problem was that it hit an iceberg, and a third person could say that the problem was it filled with water. We can write down these two "problems" on the first line. On the Titanic issue we capture the date as April 14th, 1912 and the times of 11:40 PM when the iceberg was hit and approximately 2:20 AM on the morning of the 15th when the ship was under the surface.

there are three things that went wrong with the Titanic. The most important thing of all that went wrong was that the titanic hit the iceberg. another is if the captain wouldn't made the titanic go at full speed ahead the titanic couldn't even have the iceberg in sight.

There were not enough. And many of the lifeboats they did have were launched without being completely filled.

How did people in Britain react to the US inquiry?

It was often felt that the Americans had no right to subpoena Titanic's crew and conduct an investigation, but it's an unfair opinion. The US owned the Titanic, but the Titanic was run by British safety standards. The US inquiries wanted to find exactly what went wrong, who was to blame, and what could be done to improve ship safety, while the British wished to find out what went wrong, but continue with the shipping business. It was felt by the British that it wasn't any of America's business.
