Sailor Ceres of the Sailor Quartet stated that Cosmos must be the "ultimate future form of Sailor Moon," but that statement in itself is ambiguous, as it could refer to a descendant of Sailor Moon (similar to Sailor Chibi Moon), a future incarnation of Sailor Moon (much like Princess Serenity is a past incarnation), a Sailor Moon from an alternate future, or a new form of Sailor Moon following her known future form as Neo-Queen Serenity. So it's possible that Sailor Cosmos' name is actually Usagi Tsukino, but she comes from a future that is so ravaged by war that identities don't matter anymore.

During her time, there was a great war between her and the good and Chaos. When she couldn't stand the pain any longer, she went back into the past as Chibi Chibi, AKA Sailor Chibi Chibi Moon to help Eternal Sailor Moon defeat Chaos. She reveals herself later in the story, calling herself a coward. (Anime): Sailor Cosmos is the ultimate form of Sailor Chibi Chibi Moon, who is actually Galaxia's starseed.
