Nagini: was always close to Voldemort but was slayed by Neville in Hogwarts

Tom Riddle's Diary: was in hogwarts, ginny weasley found it and harry destroyed it in the chamber of secrets with a basilisk fang

The ring: Dumbledore found it in the ruins of voldemort's mother's house in which she grew up in

And Harry: harry sacrifices himself and no longer is able to speak parcel tongue due to the fragment of voldemort's soul being destroyed

The Ring was hidden in the Gaunt family house, under the floor boards. The Diary was given to the Malfoy's for protection, though Lucius did not know it was a Horcrux. Helga Hufflepuff's Cup was hidden in Bellatrix Lestrange's vault at Gringotts. Salazar Slytherin's Locket was hidden inside of a cave with many protective spells around it. Rowena Ravenclaw's Diadem was hidden in the Room of Requierment. Harry Potter was a Horcrux that Voldemort hadn't known about and was hidden inside of Harry. Nagini was Voldemort's snake, whom he always kept neear to him, especially during the Second Battle of Hogwarts, during which, Neville Longbottom beheaded Nagini and destroyed Voldemort's final Horcrux, allowing Harry Potter to kill Voldemort.

The horcruxes were as follows:

The locket - the fake was in the basin in the Half-Blood Prince but the real one had been stolen from Sirius' house by Mundungus Fletcher, but he had in taken from him by Dolores Umbridge. Harry, Ron and Hermione took the Poyjuice potion to turn into three ministry of magic employees and snuck into the ministry to get it. Ron destroyed it with Gryffindor's sword.

Hufflepuff's cup - Harry, Ron, Hermione and Griphook went into Gringotts in disguise and stole it from Bellatrix Lestrange's vault. Hermione destroyed it with a basilisk fang during the Battle of Hogwarts.

The diary - Lucias Malfoy put it in Ginny's cauldren before the start of term. Ginny became emotionally close to the diary and was possesed by Tom Riddle. In the Chamber of Secrets Harry destroys it with a basilisk fang. At this time he doesn't know about horcruxes.

The ring - Dumbledore ventured to the Gaunt's old home and collected it, also picking up a nasty curse which made his right hand shrivel up. He destroyed it with the sword of Gryffindor.

Nagini - At the Battle of Hogwarts, Neville kills the snake with Gryffindor's sword.

The diadem of Ravenclaw - Harry found it in the Room of Requirement while searching with Ron and Hermione. Draco Malfoy, Crabbe and Goyle corner them and Crabbe accidentally destroys the horcrux using Fiend Fyre, getting trapped and dying too. Harry, Ron, Hermione, Malfoy and Goyle escape.

Harry - He surrenders to Voldemort to stop his friends dying, and Voldemort uses the Avada Kedavra curse on him. The curse does not work, Harry just falling unconscious and Voldemort falling to the ground. While unconscious, Harry sees a shriveled up creature which is the part of Voldemort's soul that was inside Harry. He also talks to Dumbledore (in his mind), who tells him that he can go on, or go back, and also ties up a few loose ends of things that we hadn't discovered.

Gaunt's Ring found by Dumbledore in the Gaunt household.

Regulus' Locket thought to be found in the seaside cave but was in Number 12 Grimmauld Place until stolen by Mundungus Fletcher and confiscated by Dolores Umbridge and finally retrieved from her in the dungeon courtroom at the Ministry of Magic.

Harry's soul horcrux destroyed by Voldemort in the Forbidden Forest. Originally not meant to be a horcrux but made when the baby Harry defeated Voldemort due to his mother's sacrifice and this horcrux is what makes Harry able to see Voldemort's thoughts and understand Parceltongue.

Hufflepuff's Cup found in the Lestrange Vault in Gringott's Bank.

Ravenclaw's Diadem found in the Room of Requirement on top of a bust of an ugly warlock. Interestingly, this horcrux was mentioned in an earlier book, have fun trying to find it.

Nagini found by Voldemort's side when believing Harry to be dead releases her from the protective charm and is killed when Neville Longbottom uses the Sword of Godric Griffindor to slay her.

And then finally, Voldemort's last remaining piece of soul residing in his body.

The diary was given to Ginny Weasley by Lucius Malfoy who hid it inside a book. Lucius has previously been given the diary and told to keep it safe but due to the increasing number of Ministry raids on his home he wanted rid of the diary before it was discovered and it was revealed that he was hiding something for Lord Voldemort. Ginny was then possessed by the memory of Tom Riddle and, through him, opened the Chamber of Secrets. When she attempted to get rid of the diary it was discovered by Harry until she found out and promptly stole it back fearing Tom Riddle would tell Harry how much she liked him. Ginny was then later taken into the chamber and Tom Riddle began to steal her life. Harry managed to stab the diary with a Basilisk fang, destroying it and saving Ginny.

The ring was left in his uncle's house and later discovered by Albus Dumbledore who, because the ring also contained the resurrection stone, put it on his finger which cursed his hand. Dumbledore later stabbed it with the Sword of Godric Gryffindor.

The locket was hidden inside the cave by Voldemort with the help of Kreacher the house elf. Kreacher told his master, Regulus Black, what his task was and Regulus decided he couldn't support Voldemort. So he stole the locket - giving his life in the process - and told Kreacher to ensure it was destroyed. However, even house elf magic couldn't destroy the locket so it sat in number 12 Grimnauld Place for years until the Order of the Phoenix cleaned the house out and the locket was thrown away. Kreacher then rescued it from the bin only to have it stolen by Mundungus Fletcher who, when attempted to sell it, had it taken by Dolores Umbridge. The trio eventually stole it from Umbridge at the Ministry and destroyed it with the sword.

The cup was hidden in Bellatrxi Lestrange's vault. The trio broke in and later destroyed with a Basilisk fang.

The Diadem was hidden in the Room of Requirement when it was the room of hidden things. Harry had seen it the year before when he hid the potions book but only realised after he spoke to Rowena Ravenclaw and heard her story.

Harry Potter and Nagini were also Horcruxes. The one inside Harry was destroyed by Voldemort's killing curse and Neville decapitated Nagini.

The Diary was given to Lucius Malfoy before Lord Voldemort disappeared. Lucius Malfoy wanted rid of it and so slipped it into Ginny Weasley's school books in 1992. She then tried to discard of it and Harry Potter came across it. Harry then destroyed it in the Chamber of Secrets.

Marvolo Gaunt's ring was found in his shack by Albus Dumbledore in 1996.

Salazar Slytherin's locket was hidden at a cave. Regulus Black then discovered what Voldemort was doing and retrieved it. He gave it to Kreacher and ordered him to destroy it. Kreacher failed to do so and hid it inside a cabinet at 12 Grimnauld Place. Mundungus Fletcher stole it and attempted to sell it but was stopped by Dolores Umbridge who took it as a bribe to let him sell his other items. Harry, Ron and Hermione then broke into the ministry and stole it from her in 1997.

Helga Hufflepuff's cup was hidden inside the Lestrange family vault at Gringotts in 1998.

Rowena Ravenclaw's diadem was hidden inside the Room of Requirement in 1998.

There was also a Horcrux that was part of Harry Potter's soul and Nagini the snake's soul. These were both destroyed in 1998.
