We had the same problem, and our 1998 F150 needed a new instrument cluster.
The old headliner and glue has to be removed from the headliner backer. New headliner material is then glued to the backer. The backer may need to be removed from the van to replace the liner.

It is a thin knit fabric bonded to a layer of polyfoam. You cannot replace just the fabric, you must remove all the old fabric, scrape off all the remaining foam and glue a new piece of headliner fabric to the hard shell beneath.
take it to an upolstery shop as more than likely the front or rear glass has to come out to get the old headliner out and the new in.
