In the 13th episode of the first season -- "The Bat Jar Conjecture" -- it is noted that Leonard was 24 when he got his PhD. In the 22nd episode of Season 3 -- "The Staircase Implementation" -- Leonard notes he first met Sheldon "seven years ago." Depending on how much time passed between Leonard getting his PhD and moving to CalTech, he would be in his early 30's when this series starts.

A typcial medical doctor probably doesnt have a PhD. Most doctors have MD degrees instead. Some medical doctors have done MD/PhD programs, but they primarily prepare candidates for research instead of practice
