When the Emperor was electrocuting Luke and Luke was close to the point of being electrocuted to death, Vader then became Anakin again at that moment. Vader then picked the Emperor up and threw him into a power generator which killed the Emperor. But before he was thrown, his Sith lightning hit Vader's life support systems and he died shortly afterward. But he died as his true self and not as Vader.
