Each of our political parties elect a leader orreaffirm their current leader at a party convention. When an election is called each party leader runs in their own riding and nation wide. After the votes are in, each riding counts the ballots and the win goes to the person with the most votes. At the end the party with the most elected candidates wins and the leader of that party becomes Prime Minister. The leader of the party with the second most votes becomes the Leader of the Official Opposition.
A motion of non-confidence does not "vote out" the prime minister, it only requires that the prime minister either resign (in which case a new prime minister would be summoned immediately by the Governor General of Canada), or seek the dissolution of Parliament (in which case the prime minister would continue at least until the results of the subsequent general election).

In Scotland or any part of the UK you do not vote for a Prime Minister. You vote for your local Member of Parliament. He or she is usually a member of a political party, and the party elects a leader who becomes the Prime Minister if that party has the most Members of Parliament.
If the Prime Minister dies, the governing party selects a replacement. In some countries this person must then be approved by a monarch or president.
