Jesse was expecting it was the russian orderly/pimp when he said "Don't turn around Valerie" but when he removed the night vision mask, it turned out to be Lewis the young clean cut man working at the clinic who gave Jesse the info about Valerie.
The killer is just a "no name" contract killer who is himself shot and killed by Jesse Stone at the end. The killer was hired by Hasty Hathaway, who, as it turns out is really a criminal mastermind and has been the REAL mob boss all along. Hathaway was even running his criminal enterprise while in prison and is really Gino Fish's boss and not the other way around.
