So the major difference between the two is the location, and characters. Resident Evil 5 occured in Africa, and pairs Chris and Sheva against Wesker, in the movie Sheva is replaced by Chris's sister Claire, but all involved still face Wesker, and the film mostly takes place in LA. The las plagas creatures are in the game but are explained to be further zombie mutation, and not a parasite like in the game, the Axeman makes an appearance as well as the las plagas dogs like in the game. Jill also appears in both as being controlled by Wesker with the spider like device, but is not as involved in the movie only appearing in the final scene. Another difference is Wesker dies in the game, but not in the movie, you see him survive if you pay close attention to the scene where Luther comes from the drain.

He's the final boss in the game which you can finally have a one on one battle. And you get to see his demise. I don't think he finally dies although there is a big explosion, it only makes him weak because if he was dead they wouldn't be creating resident evil 6.
