Tuxedo Mask was brainwashed by enemies in the first and fifth seasons of the anime. In the first season, he was brainwashed by Queen Beryl shortly after he recovered his memories as Prince Endymion and remembered the truth ("turned back good") in the final episode of the English-dubbed first season, episode 40, "Day of Destiny" (which combined portions of Japanese episodes 45 and 46). In the fifth and final season (which was never dubbed into English by any official organization), he was brainwashed by a revived Queen Nehellenia to forget Usagi. After a long struggle just to get to him in Queen Nehellenia's illusory palace, the curse is broken and Mamoru remembers who he is, in episode 172, "The Power of Moon Love! The End of the Nightmare."
Yes. This is revealed for the first time in episode 83 of the original Japanese anime (episode 76 of the English dub) and in the manga in Act 20 "Crystal Tokyo, King Endymion." In the anime, it is King Endymion who explains it directly to Sailor Moon and Tuxedo Mask while Rini is sleeping, but in the manga, Rini actually introduces herself as the daughter of "Neo-Queen Serenity and King Endymion" but Sailor Moon and Tuxedo Mask don't quite understand until King Endymion clarifies that Neo-Queen Serenity is a future form of Sailor Moon herself--and therefore that Rini is Serena's own future daughter!

In the original Japanese episode 26/English dub episode 22, Mamoru [Darien] awakens from a dream where he is Tuxedo Mask. He wonders if he is Tuxedo Mask, and gets his answer later in the episode, after he acquires the Orange Rainbow Crystal. After the events of the first season, Mamoru loses his memory of being Prince Endymion or Tuxedo Mask. However, he regains his memories, including those of the identity of Sailor Moon, in Japanese episode 59/English dub episode 53. From that point forward, he only appears as Tuxedo Mask or Prince Endymion, never again as Moonlight Knight.
